2012 LHPS Jr. Internship Site Proposal Form
Please submit a separate form for EACH WORKPLACE where you will job shadow as soon as possible, but no later than Friday, February 24, 2012!
Please note that the form will only submit after each box is completed! Submitting the WORKPLACE CONTACT INFORMATION indicates that the WORKPLACE CONTACT has already authorized for you to job shadow at their location.
** IMPORTANT: Please note that if this is an Orlando Health facility, additional paperwork and different deadlines apply.**
