Read this note from our Athletic Trainer's office:

PARENTS: A current physical and liability form is required for any athlete to participate in sports at Lake Highland. All paperwork must be on file and current in the athletic training office at least 24 hours prior to the first day of practice/tryouts before any athlete is eligible to participate or tryout for any team. Copies of these forms can be downloaded here: Consent Form & Physical Form. Please be sure to download BOTH forms and return completed forms to the Athletic Training Room.
