I hereby declare my son/daughter is in good health and able to participate in the summer activities. I will not hold the LHPS administration, faculty, staff, or volunteers liable in case of injury as a result of participation. I hereby authorize the staff of Lake Highland Preparatory School to act on my behalf, using their best judgment, in case of any emergency requiring medical attention for my child in the event I cannot be contacted. It is understood that I will be fully accountable for any and all costs incurred in the treatment of my child. I understand that if a minimum number of students do not register for a class section, I will be notified and alternative class choices will be discussed. The receipt of the electronic confirmation does not guarantee admission to class or availability of the class. I understand that, unless otherwise stated, a 20% processing charge will be assessed for withdrawal from each class. If Lake Highland drops a class or if a student is not eligible for a class, a full refund will be made of all fees collected. No processing charge will be assessed for changing from one class to another. I understand my monthly statement will be billed.

I understand that all assignments must be submitted electronically on the dates due and that any on campus tests must be taken on one of the scheduled dates. All materials and hard copy projects must be turned in on the test day. I will have Internet access for the duration of the course. I understand that I will be dropped from the course if I miss more than two assignment deadlines.
