The Lake Highland Athletic Department is hosting its’ annual Sport Physicals on May 18th 2010
Location: Athletic Training Room/Gymnasium
Time: 3:30pm
Cost: $15
Parents please have athletes bring signed consent form, Impact form, and physical form to be submitted at time of physical.
Paperwork is located on the athletic training website.
This is the same price, or less, as most insurance co-pays. It would be beneficial, and is strongly advised, to have your child present on this day so that you may make sure they will be eligible for all activities for the following calendar year. This physical will also cover your child during the entire 2010-2011 academic year.
